Transcription: I James Usher formerly known as James Munday Private No 999 of the 2nd Battalion Duke of Cambridges Own Middlesex Regiment now residing at Kamptee in British India do make oath and say as follows: - 1. That at the time of my birth my parents name was Usher as appears from the Baptism Register of the Parish of Freshford in the County of Somerset in England. 2. That subsequently my mother was married to one Munday and I was and known by the name of James Munday [by] which name I enlisted in Her Majestys Services. 3. [** torn section] that on the 29th day of November [folded section] ** the ** of James Munday and was [married to] ** [at] Christ Church Madras. 4. That on the 19th day of August 1885 my son James Henry by the said marriage was baptized at Secunderabad in British India and my surname in the Register of such baptism is given as Munday. 5. That I have lately reverted to the use of the name Usher as my surname and it is my intention and desire that I and my said wife and my said son and any other children of mine may and shall henceforward use and be known by the surname of Usher and not by that of Munday or Mundy. 6. That for the effectuating of such intention and desire I have made this my affidavit in seven fold that one copy thereof may be annexed to each of the three copies of the entry of my said marriage which are now extant viz one copy to the original Register Report at Christ Church Madras one copy to the Register Report in the office of the Madras Diocesan Register and one copy to the Register Report in the office of the Government of Madras.
